Professional Association participates with most insurances. If
you would like to schedule an appointment, please call our
office at 856-696-2010.
Our office will make every
attempt to accomodate your scheduling needs. Our computerized
appointment system allows us to quickly find an open
appointment.
You will need to supply us
with information at the time the appointment is scheduled. We
will need to ask you for your address, phone number, insurance
information and why your are being treated.
If you are being treated for a
work related injury, we must have the authorization from your
employer or the compensation insurance company.
For auto related injuries, we
will need the claim number for your accident. You will be asked
to bring the insurance information with you since New Jersey is
now managed care and requires most procedures to be authorized.
All patients with an HMO
insurance will be asked to obtain a referral from their Primary
Care Physician.
Depending on why you are being
treated, you may be asked to bring any x-rays or MRI studies
with you for your initial appointment.
If you have any special needs,
please inform us prior to your visit. We are handicapped
accessible.
